Anyone will be allowed to enter a team, but, not all teams will be accepted
Please make sure you follow the notes below, and you must first communicate with Kelly Katorji. A team entry is responsible for jerseys, coaches, etc, it is your team.Step 1: Email Kelly Katorji at kelkato@rogers.com with interest to submit a team entry.
Step 2: Please download this team registration form |DOWNLOAD|, fill it out, and email it to Kelly Katorji at kelkato@rogers.com ASAP. It is an excel spreadsheet so you will need excel
Step 3: Kelly Katorji will confirm with you acceptance of your team
Step 4:Please download this waiver form |DOWNLOAD|and pass out to your players, as each player must fill one out and give it to you to bring to the event, it is a very basic form (excel format)
Step 4: Payment and all paperwork, including a hard copy of the Individual Waiver form must be submitted (due dates and other will be emailed to you upon contacting Kelly Katorji)PLEASE EMAIL KELLY KATORJI FOR TEAM ENTRY COST

If you are qualified to play in this event, and you do not have a team, there will be teams put together so you have a spot to play. Follow the steps below. Please note, in this case, your entry fee will include jersey and coaches and insurance. The team will be put together for you all you will need to do is show up and have a good tournament.

Step 3: Kelly Katorji will confirm and make sure you have a team to play on (please note, only upon confirmation are you guaranteed into the event, so please make sure you get confirmation verbally or via email from Kelly Katorji)

Prospects needs the name of your insurance company, the name of your insurance broker, your policy number and the policy period.
